Bad breath is caused by bacteria. Every ones mouth harbours millions of bacterial cells of many types.

The type of bacteria that cause bad breath are often protein eating anaerobes, meaning they thrive in a low oxygen environment.
This means they hide in inaccessible parts of your mouth, deep in gum margins, where they can cause periodontal (gum) infections also. Often they will hide in the deep folds at the back of your tongue. Removing them is the best way to get rid of bad breath, but this can be difficult as they are hard to get to. Often only the dentist can clean around all the spots these bacteria hide in.
Brushing and flossing is usually a good place to start. Scraping the back of your tongue with your toothbrush is surprisingly effective at removing bacteria from this area.
An oxygenating (bubble producing) mouthwash can help to kill anaerobic bacteria.
A dry mouth can predispose you to bad breath because the bodies natural antibacterial agents in saliva don’t get a chance to work. This can be a problem in elderly people and those on lots of medication. So drinking plenty of freshwater will also help.
